Succesful Case Summaries

 LIMB SALVAGE

1.     23 year old gentleman came to us with osteosarcoma left superior and inferior pubic rami. He had already undergone one surgery for the tumour in south africa. But the tumour reccured. The tumour was along with the sorrounding soft tissue. All the nerves, vessels and other vital structuresaround that area were preserved. The gap between left hip joint and pubic symphysis was bridged with a 6 inch fibular graft.
The graft was fixed with screws. The surgery lasted 8 hours and it required 16 blood transfusions. Patient was able to walk in partial weight bearing with support after 1 week of surgery.
As the patient has localized tumour ( no metastasis) this procedure is radical treatment for the tumour.

2.     45 year old lady came to us with carcinoma breast with secondaries in left femur ( upper half).
It was very painful and she was bed ridden. After consultation with oncologist her left                       "Hip replacement with proximal femoral replacement"  was done. She was able to walk with walker support after 3 days and after 1 month she started walking without walker. The tumour with sorrounding soft tissues was excised after 1 year there is no pain and no reoccurence of tumour.
Now she has come with pain in right hip , PET scan shows a lesion  in the floor of acetabulum. It was excised , the cavity filled with bone graft and a standard non cemented hip replacement was done. As the bone quality reasonably good, we were able to fix the acetabulur cup without screws. 48 hours after surgery patient is walking with walker and with little pain.

 ACL and PCL RECONSTRUCTION

Mrs Neena chopra , 37 year old lady fell down from stairs . She had severe pain and swelling in right knee, she was unable to walk normally.
On Examination there was gross instability of the right knee. Mri showed complete  ACL ( anterior cruciate ligament ) and PCL ( posterior cruciate ligament) tear. Both the ligaments were reconstructed using patient's hamstrings tendon. The procedure was done Arthroscopically in a single sittimg.
Patient started non weight bearing walking from day 1 of surgery. After 6 weeks patient started walking full weight bearing with brace and the Range of motion was 0 - 90.
In the recovery phase all the muscles were strengthened with emphasis on Quadriceps, Hamstrings, Abductors, Gluteals and Core muscles as a result of which  Patient has recovered well.
