Degenerative Disc Disease

70 years old gentleman from Rohini Delhi was suffering from severe backache for last many years. The pain has recently started radiating to both his legs. He has difficulty in sitting as well as walking. MRI showed degenerative disc disease at L4-5 Level causing instability & compression of nerves. He was refused at various centers because the patient was suffering from Diabetes, Hypertension, Bronchial Asthma & Chronic Kidney Disease.
Performing a major open surgery in a patient like this having so many systematic diseases is definitely a big challenge. In open surgery, we have to cut the muscles and bones leading to bleeding and stress on the body systems. Even the healing takes longer and patient has to be bed ridden for many weeks. To avoid all these hassles, we decided to perform Endoscopic (MIS) surgery in this patient.
Surgery lasted about two and half hours, there was no need for blood transfusion and patient was able to stand on day one. This shows the distinct advantage of MIS technique in spine surgery.

60 years old lady from Karol Bagh, New Delhi, presented to Dr. Ashwani Maichand with complaints of low back ache & shooting pain left leg. She was hardly able to walk and complaint of tingling sensation in left foot. Her MRI showed degenerative disc disease at L5-S1 level with pressure on left S1 root.
Endoscopic distectomy was  done and L5-S1 fixation performed by Percutaneous technique. The surgery lasted around 3 hours, no blood transfusion was required and her post op period was very comfortable.

 Case 3

55 years old man came with severe back pain and inability to stand and walk without support. There was some numbness both legs but there was no urinary problem. He was admitted through emergency but pain could not be relieved even with injectable medicines. MRI showed advanced degenerative changes at L1-L2-L3 level. It caused gross instability of spine.

Dr. Ashwani Maichand & Team performed Endoscopic decompression of the spine was done and two level (L1-2 & L2-3) fixation was done by percutaneous technique. Percutaneous technique means that we don’t open the spine at all. All the muscles and bones are preserved and there is no bleeding. Next morning, patient was walking without pain and without support. The best part was that such a massive surgery was done without any blood transfusion.
