50 years old police officer from Bahadurgarh (Haryana) had a spine surgery 5 years back for slipped disc. He was doing alright after the surgery but in April 2016 he developed a similar pain again. He was not able to go to washroom due to Sciatica Pain. His left leg was feeling numb and whenever he turned in bed he had shooting pain upto toes.
MRI showed a slipped disc at L4-5 level and Meningocele at the same level (Dural Tear due to previous surgery). Due to first surgery there was some damage to the dural layer (protective layer around spinal cord) which caused Herniation of CSF Fluid into surrounding muscles.
The challenges in this surgery were :
1.       Removal of disc without further damaging the dura.
2.       Control of CSF loss during surgery.
3.       Dural Repair.
4.       To perform all this Endoscopically.
Endoscopic dural repair is a very rare &n tedious surgery because the field of vision is repeatedly Obscured by persistent CSF & you cannot put stitches in dural through this limited approach. So Dr. Ashwani Maichand & Team prepared a muscle plug from patient’s body and pushed it through the dural tear in such a way that it acted as a Valve.
Patient was asked to be on bed for next 3 days, on 4th day when patient was mobilized he had no pain, there was no CSF leak and there was no need for any walker / stick for support.

  • Mega campaign on Arthritis Management in association with "Varishth Nagrik Kesari Club" - Punjab Kesari Delhi on 6th April 2016 (Wednesday)

  • Dr. Ashwani Maichand Participated in Shoulder Conclave 2016 at JWT Marriott, Pune

  • Dr. Ashwani Maichand was the Table instructor for young doctors at Cadaver Joint Replacement Training on 21st February 2016 at All India Institute of Medical Sciences (AIIMS)
