Total Hip Replacement after failed PFN

75 years old gentleman had a fall in washroom 2 years back. He sustained fracture right hip for which fixation with PFM (nail & screws) was done elsewhere. Patient was never pain free, he had to take painkillers everyday and had to use walker for support.  As the fixation was not adequate for this type of fracture, the nail was broken after 1 and half year and patient got bed ridden. The patient had additional problems like his Cardiac by-pass surgery was done 5 years ago, he is on pacemaker, he has CKD (chronic kidney disease).
His serum creatnine is 6.3 (maximum normal is 1.4), hemoglobin is 7.4, cardiac function is only 25% (ejection fraction) and his limb is 2 cm short. He has not walked for last 6 months.
Challenges are:
§ Severe Osteoporosis (because patient is bed ridden for 6 months)
§ Poor muscle power ( because he is not using this limb properly for last 2 years)
§ Loss of bone in upper part of femur ( due to previous implants)
§ Broken nail in femur (very difficult to take out)
Dr. Ashwani Maichand & Team in consultation with cardiologist and nephrologist decided to go for hip replacement. All the old broken implants were removed, non cemented hip replacement was done by MIS technique. The surgery and post op period was uneventful. Patient had to undergo dialysis before and after the surgery. 3 units of blood was transfused but thanks to our anesthesia and ICU team that patient came out unscathed.

He was made to stand with a walker day one took his first pain free steps in two years after two days.
